got risk assessment? dont have...up lorry

 

got risk assessment then morning got tool box meeting? dont have...up lorry

 

got risk assessment and toolbox meeting then got permit to work? dont have...up lorry

 

everything have....now who the PE, material supplier, erector.....

Pls lah the whole risk assessment is a joke.....do for the sake of doing..... paperwork....

 

Just because it is recommended by some top guy, they die die have to make this a success..

Risk Assessment is a very useful tool, if done properly and by competent person. For example, how does a worker know the hazards and risks associating with his/her work, if there are no risk assessment (which also covers control measures to be taken)?

 

It is a sad thing that many company and safety officer take RA too lightly, treating it more of a paper work (just like what many management thought of ISO certification) than a effective tool to help them in managing occupational safety.

 

If you are in this line before, you will know what I means. ;)

Just curious, are you in safety line, or ever involved in accident investigation? FYI, even if got RA, but not done by competent person, also up lorry... Actually scope of work and SOP are equally important, especially in such high risk industry.

If lah..... the current MOM recommendation of quantitative RA is rubbish... High Medium low...... WTF is that.

High Risk - chia more lui $$$$$ aka cover both eyes & ears

 

Medium Risk - 50% discount from that high risk aka cover both eyes only (ears still can listen)

 

Low Risk - kopi lui can leow aka cover 1 eye (kum jit bak)... -_-

one engineer commented that it could be due to large amount of cement pour onto one area instead of spread out evenly, and if the loading area cannot take the weigh of the cement then it collapse.

 

It could still be human error and wrong calculation.

If you read carefully the reports, 2 things stand out

a. They group of workers are at the end of their night shift. After pouring the concrete, they will be off and hand over to the morning shift

b. They have been doing this for many times. In fact, this is the last section. Once done, the entire structure wuld have completed

 

So, 2 conclusion I can draw

a. fatique... After working whole night, very tired already. Just like last time our controller kept reminding us to be careful after exercise cut and all so eager to go back camp. Very high occurrance of equipment lost and personal injuries. I still remember during one ICT, my section mate whole night jump up, jump down the tonner for mission bo dai chee. Then exercise cut and the very next jump down from the tonner, he ended up in Khoo Teck Puat. And he ended his ICT a few days earlier than the rest of us

 

b. Complacent.... Done this many times. Every time also steady steady pong pee pee.... So, cut corners here and there. Ended up the corners became a chamfer and hence accident happened.

Have not read the report yet. But basing on preliminary info from press and MOM, a few areas the investigators will have to look into:

- Loading of scaffold (PE would be the best person to answer this)

- construction of scaffold (soundly constructed, using the right material and in accordance to PE design?)

- Usage of scaffold (is the load evenly distributed? any violent shock when pouring the cement at rate too fast?)

- No. of people allowed on the scaffold (by law it stated max. 8 pax per bay)

 

Going back to RA, a good RA shall covers all hazards and address all risk, including worker and environment conditions. Of course whether the worker follow the control measures documented is another issue, and responsibility shall be on the supervisor, manager, etc.

MOM uses a 3 by 3 matrix, which is the min. standard for risk rating. There are companies already using 5 by 5 matrix...
