Explosion at shipyard

Shipyards and refineries are one of the most dangerous industries to work in. Whatever precaution we made, just need 1 mistake to "blow" everything.

 

That's why my colleague is sweating over a system installation in the coming months at a refinery. [sweatdrop]

 

Hope that there are no fatalities in this incident.

 

Not as terok as he thinks it to be lah.

Eyh, yard cannot bring cameras lah. I'm told cannot go "visit" the site. Off limits and under investigation. KNS!

That's what I'm trying to say. We can take all precaution we want but just need someone to make a mistake.

Moreover that refinery just have an accident which someone died.

An accident happened at Benoit Road involving a barge belonging to Haosen Marine Pte Ltd.

 

A spokesman from the Singapore Civil Defence Force (SCDF) told AsiaOne that two workers were pronounced dead when an SCDF team arrived on the scene.

 

The Police also confirmed they received calls about the explosion at about 1.30pm and rushed their personnel to the site.

 

There are a number of shipyards along Benoi Road, including Keppel Singmarine, ST Marine, UOL Marine and Asia Pacific Shipyard.

 

Investigations are ongoing.

Go toilet pang sai also can die, bowl crack, cut backside, bleed to death. Serious, got this case happen before.

Play golf, kana lighting, play golf, go toilet, backside kana bitten by snake? Why worry so much?

 

If like that, me everyday stress liow, coming to work, some more, my wife is working in my.."sister" yard! -_-

Depends -

 

Generally every "accident" is ultimately human failure. But it also depends on the safeguards built into the system, and whether the supervisor creates an environment of adhering to systems.

 

Normally it is about active threat identification (which is inherent) and then seeing what can be done to minimise or circumvent threat.

 

Take in the case of traffic (for example) - in a cross junction

1. Threat identification would be traffic going in different directions meeting

2. Threat minimisation would be traffic lights

3. If there is an accident it means one of two things

a) Somebody ignored a red light

b) the system screwed up and both sides green at same time.

 

Now, if someone ignored a red light it could be further tracked down

a) is the transition from green to red too fast for the speed the cars travel on the road (and was this known)

b) Were the lights clearly visible

c) was the driver just not paying attention

So you can see that even in the case of "the system failed" there are futher reasons for WHY the system failed and the accident happened.

 

Last time when I was a safety inspector as part of my duties at the work place it was all about identifying the risk first, and then seeing what can be done to remove the risk.

[laugh] Cannot blame my colleague lah......it's our first refinery customer and it's his first time stepping into one. Moreover the news of the accident shook him a bit lah. [laugh]
