Freak incidents

Often we momentarily forget about the spinning fan that is low enough to hit. Not too long ago a two year old girl died when she hit the ceiling fan while playing with her uncle in  Malaysia.

Frankly, such big ass fans and its setup are very safe, even safer than the normal ceiling fans as some of them have to be endorsed by PE (Professional Engineers) due to their weight and the location of installation.

All this type of work will not have procedure for Risk Assessment,  Tool box meeting etc....

Sad to say with a very lean team to deal with all these repairs and maintenance, RA were often overlooked and even impractical to be implemented on the ground. I have lost count of how many maintenance activities I have to intervene and have the approved permit to work cancelled, despite the complaint from the stakeholders...

Yes, and in the simplest term, it is about situation awareness. We can argued till cow come home about the competency of the worker, lack of RA and supervision, but end of the day if they worker just work and/walk around blindly (akin to crossing a busy road without keeping a look out for traffic), accidents will happen. 

And that applies to passerby as well. If any of the patrons at that coffee shop keep a active look out for what's going on around them, maybe the guy wouldn't even have a chance to lift up his ladder... I don't just talk, I act on unsafe practices always...

Well said. Those eating in the coffeeshop if paid attention to the worker and remind him can surely prevent thus accident. 

I once also remind a FW working on height  to hook up the safety belt on his body to the anchor point. No supervisor around them and a fall can be fatal.

Not to start another new thread, accidents like this is not freak but man-made. I thought I wish to share my observation here. Personally, I find these grease interceptor covers were made of very poor quality. The metal sheets were too thin to withstand a human weight. Most were already bent waiting to give way. This is also very commonly seen on drain covers over walkways. Each time a person jog or walk over it, the cover will wobbles and create noises. Don't we have some basic specification to ensure a minimum standard for such covers?

Interestingly every time a report like that comes out, how quickly and distinctively the buck gets passed to the coffee shop owner by the Town Council.

Grease interceptor required NEA inspection and clearance. I wonder how they can pass the installation with such poor quality cover. Just visit one of the hawker centre and try walking on top of one of these cover to feel the quality.

Maybe the work procedure for these agencies is to close all eyes during inspection? 😓

In some places, concrete slaps were used. These metal plates are quite sturdy. The fault here lies with insufficient support below the plates. The narrow end of the plate did not have full support, thus, it slipped and gae way when weight is loaded upon it.

I too would do the same. Once I saw some fts doing electrical work by the road side, leaving the control box door open with live wires. I told them to close it!

To overcome the think material used, they added the metal support but insufficient. Those drain covers are without any. The inspection chambers (IC) were there for many years. It never have this problem because the material used is correct.  It is about time some basic standard be set.
